The Academic Foundation Department (AFD) of The University of Cambodia would like to inform those students who enrolled with AFD in Promotion 8 Term 2 Academic Year 2009-2010, that AFD is doing the processing in order to issue the Foundation Year Certificates for those who will successfully complete the their programme then. Students are requested to get the Clearance Form at AFD, fill in all required information and submit it back to AFD by January 8, 2011 at 5pm.
Requirements
1- Verify your personal information with Bac II Certificate (or other official equivalent document)
2- Write your name clearly in Latin.
3- Attach 2 photos, size 4Í6 (new photo less than 6 months).
AFD is not responsible for late submission or failing to submit the form; or for any misinformation you have made by yourself for issuing the AFD certificate.
